CollabraSpace Benefits Summary

Health Care: We offer a Health Savings Account (HSA) with both Blue Choice HMO and Blue Preferred PPO. CollabraSpace continues to contribute 100% of the health insurance premiums for employee, employee and spouse, employee and child(ren), or employee and family.

Dental Care: The company will offer each employee a dental plan through Guardian to the employee and their family, as well as children’s orthodontia. The company will pay 100% of the premiums.

Long Term Disability Insurance: Each employee will be provided long term disability insurance through Harford that is based on an individual’s salary. The company will pay 100% of premiums.

Short Term Disability Insurance: Each employee will be provided short term disability insurance thorough Hartford that is based on an individual’s salary. The company will pay 100% of premiums.

Life Insurance: Each employee will receive one times their salary or a minimum of  $100,000 (whichever is larger) worth of life insurance.  The company will pay 100% of the premiums.

Voluntary Supplemental Life Insurance: CollabraSpace participates in the Voluntary Supplemental Life and AD&D Insurance plan offered through The Hartford.  Once eligible, through payroll deductions you can purchase in increments of $10,000 up to 3 times your salary or $300,000 maximum, with a $30,000 minimum guarantee.

Voluntary Vision Coverage: Each employee may participate in vision services at a nominal cost. The plan includes routine eye exams, eyeglasses, and contact lenses offered by CareFirst BlueChoice, through VSP.

Dependent Care Flexible Spending Account (DFSA): We offer each employee the opportunity to participate in contributing pre-tax funds to an account that covers child or elder care services.

401k Plan: You are eligible to participate in a 401k retirement savings plan sponsored by CollabraSpace on your first day of employment.  Contributions will be deducted from your semi-monthly paycheck on a pre-tax basis. CollabraSpace employess are 100% vested from day one of employment.  The company will match 5% of your gross salary (3% Safeharbor Match and 2% Profit Sharing Match).

Employee Stock Ownership Plan: Since each employee is responsible for the growth of the company, all employees will receive stock options in the company on their date of hire. Additional options may be granted as part of a bonus plan to reward outstanding performance.

Annual Bonus Plan: All employees are eligible to participate in the CollabraSpace Bonus Plan. When CollabraSpace meets its fiscal year financial plan, 15% of the company’s profit will be distributed to employees via a cash bonus.

Mobile Phone / Internet Connection: CollabraSpace wants each employee to stay connected. Collabraspace will reimburse each employee up to $50 per month for telephone and up to $50 per month to defray the cost of home-based broadband Internet services.

Health Club Membership: CollabraSpace encourages a healthy lifestyle. In order to encourage this, CollabraSpace will reimburse employees for a health club membership, up to $50 per month.

Educational Reimbursement: CollabraSpace understands the need for keeping skills current. Our educational reimbursement is structured to help employees meet their higher education goals on an      individualized need.

Paid Time Off (PTO): CollabraSpace provides accrued Paid Time Off (PTO) for all employees, which may be used for vacation, sick, holidays and or other days not worked for personal or family reasons.  Accrual begins with your first day on the job. All full time employees are eligible to accrue up to 240 PTO hours per calendar year.

2- Week Paid Sabbatical: Every 5 years of continuous employment with CollabraSpace, each employee is offered Sabbatical Leave benefits. Employees who have earned  a sabbatical may choose take either 2 weeks of time off or a cash payout.

Cash Payout will be equal to 2 weeks salary plus 5k, 10k, or 15k depending on the anniversary they are celebrating (i.e., 5 year = 5k, 10 year = 10k, 15 year = 15k).
